Welcome to the “How to Enroll” section of the Citrus Health Care website. Listed below is a list of the requirements you must meet in order to enroll in a Medicare Advantage plan offered by Citrus Health Care.
Requirements
  • You are entitled to Medicare Part A and enrolled in Part B as of the effective date of your expected coverage.
  • You have not been medically determined to have End State Renal Disease (ESRD) prior to completing the enrollment request.
  • You permanently reside in the service area of the Plan you are electing.
  • You complete an enrollment request and include all the information required to process the enrollment.
  • You are enrolling during a valid election period.
  • You must continue to pay your Medicare Part B premium.
  • If you want to enroll in the Citrus Health Care Special Needs Plan (PBP-011) you are eligible for both Medicare and Medicaid. Please contact Citrus Health Care Customer Service to find out how to enroll in this plan. You may call us at 1-866-796-1157 or TTY users may call Florida Relay Services at 1-800-955-8771. Our hours of operation are Monday through Sunday from 8:00 a.m. to 8:00 p.m.
 
Election PeriodsThere are four types of election periods during which you can make an enrollment request: a
  • Annual Election Period from November 15 through December 31 of every year.
  • Initial Coverage Election Period for newly eligible individuals that begins three months immediately before your first entitlement to both Medicare part A and Part B and ends on the later of: (a) the last day of the month preceding entitlement to both Part A and Part B or (b) the last day of your Part B initial enrollment period.
  • Open Enrollment Period from January 1st through March 31st when eligible individuals may make one request.
  • Special Election Period of which there are various types including, but not limited to: change in residence, low income subsidy, and dual eligible individuals. For more information on special election periods, please contact our Customer Service Department.
 
Three Simple Ways to Enroll
  • You may enroll directly from the website by selecting the “Click to Enroll Now” button below on this page. This option is not available for the Citrus Health Care Special Needs Plan. Please contact Citrus Health Care Customer Service to find out how to enroll in this plan. You may call us at 1-866-796-1157 or TTY users may call Florida Relay Services at 1-800-955-8771. Our hours of operation are Monday through Sunday from 8:00 a.m. to 8:00 p.m.
  • You may enroll by selecting the “Click to Print an Enrollment Form” button, printing a Citrus Health Care Individual Enrollment Request form, and mailing it directly to us at the address on the Instructions page.
  • You may enroll by calling our Customer Service Department to schedule an appointment with a Citrus Health Care Sales Representative. Please call us at 1-866-796-1157 or TTY users may call Florida Relay Services at 1-800-955-8771 our hours of operation are Monday through Sunday from 8:00 a.m. to 8:00 p.m.
